  • IN4-0331:INVALID PAY TO PRVDR ZIP CODE
  • IN4-0301:INVALID BILLING PROVIDER ZIP
  • IN4-0309:INVALID OTHER PAYER POSTAL CODE
  • IN4-0324:INVALID BILLING PROVIDER ZIP
  • IN4-0308:INVALID OTH SUBSCRIBER POSTAL
  • IN4-0305:INVALID ACTIV PAYER POSTAL CODE
  • IN4-0309:INVALID LAB/FACILITY ZIP CODE

This problem started on May 5, 2017 at 12:00 AM CT and was fixed on May 5, 2017 at 8:45 AM CT. RelayHealth will be reprocessing all affected claims.
