When assigning or transferring the treatment of a patient to a different practice/facility/provider, make a summary of care record.

Numerator:
Quantity of clinical encounters in the denominator that are related to summaries of care directed electronically via Direct Messaging, and those communications have been obtained and read. The Summary of Care Record Provided for Care Coordination Electronically check box is chosen on the Clinical Encounter screen, and the Read Receipt check box is chosen on the direct message that is mailed.
Particulars on what to watch for:
1. Verify that the CCDA accompanying is to the direct exchange message and all the obligatory components are in it.
2. The ACI the measure requires the CCDA attached direct exchange messages need to have the Read Receipt box chosen.
3. You should watch the replies come back, if they are unidentified then SureScripts never got a delivery reply back. If you see a Received arriving in return then the service will alter the message Return_Receipt from 'Y' to 'R'.
That is what the ACI measure wants for it to total.
