I do the billing for a podiatrist and I can not figure out where to put my referring physician NPI and date last seen, everything I have tried still gets me an ADR from Medicare I have to do for every 11055, 11056, or 11721 that I submit. please help.

If you are using Medisoft, the referring physician's NPI goes on the Referring Provider IDs tab of the Referring Provider record. Date Last Seen is on the Miscellaneous tab of the Case.

All that is already in my case and does not populate where Medicare wants it.
the date last seen needs to be in loop 2300 segment DTP and element DTP01, which should send the 304 qualifier, I don't know where to put the date so that DTP01 date/time qualifier populates on my claim.

That's where it goes. Are you using Revenue Management to send the claims in ANSI 5010 format?
